Avenue H makes insurance easy and affordable

Avenue H is Utah's small business health insurance solution. We help small businesses with at least 1-50 full-time equivalent employees control costs while offering expanded coverage to their employees. Brokers play a key role in helping small businesses register with Avenue H. Brokers are instrumental in assisting employees with plan selection and ongoing administration.

Employee Choice: Offer your employees quality health plans provided by local, private health insurance companies. All plans offered on Avenue H meet the requirements of essential health benefits.

Simplified: Employees can compare multiple health plan benefits, premiums and network side by side.

Controllable Cost: Decide how much you can afford to contribute using our flexible Defined Contribution approach. There are no additional fees to participate; the plans and premiums are the same as buying direct from the insurer.

Open Year Round: You can register your business any time throughout the year.

Training and Certification

Avenue H utilizes brokers to help educate the business community and their employees about the benefits of Avenue H, Utah’s Health Insurance Marketplace.

Under Utah state law:

  • Producer (broker) is required to have a State of Utah individual producer license for health and disability insurance. For additional information, refer to
  • Broker must have a direct individual appointment with a majority of participating medical carriers and at least one participating dental carrier that offers a defined contribution arrangement on Avenue H.

Participating medical carriers: HSA Health Plan, SelectHealth and UnitedHealthcare

Participating dental carriers: Alpha Dental, Delta Dental, Dental Select, and EMI Health

  • Broker must complete Avenue H market training annually. This training includes training on premium assistance (UPP). These classes are offered by the Office of Consumer Health Services as approved by Utah Department of Insurance. Courses are free of charge, and brokers are able to earn CE credit for attending the courses. See upcoming training sessions here.

Once a broker has met the requirements listed they are credentialed with Avenue H. Credentialed brokers are included on Avenue H website under Agent Search - Authorized which is hosted by the Utah Department of Insurance. Group Eligibility

To join Avenue H, groups must meet the following criteria:

  • You must have between 1-50 eligible employees
    • An "eligible employee" is someone who works 30+ hours per week
      • For groups effective January 1, 2016, group size count is determined by full-time equivalent (FTE)
  • At least 50% of enrolling employees must live in Utah
  • At least 75% of eligible employees must enroll
    • Employees with other coverage through a spouse/parent, government program or individual policy are excluded
  • Employers must also have a 125 Premium Only Plan

The final step of registration is to provide your banking information prior to enrollment using the Invoicing and Payment link on the secure portal.

Key Dates:

For new business, the registration process must be completed no later than the 10th of the month prior to the month of coverage. Employees will have until the 15th of month prior to the groups effective date to enroll.

For example, a group that registers for Avenue H on November 29th will have its open enrollment period Dec. 1st - 15th, and its coverage would begin January 1, 2015.

Avenue H not only handles your initial enrollment but we also handle ongoing processes for New Hires, Qualified Life Events, Terminations and Annual Renewal. You’ll be able to do all of these functions online in the same system.
